The film is based on the popular manga Cells at Work (by Akane Shimizu, serialized in Kodansha's Monthly Shonen Sirius), which anthropomorphizes human cells, gaining widespread attention for its innovative and unique concept, as well as the spin-off Cells at Work: BLACK (by Shigemitsu Harada, Issei Hatsuyoshiya, and Akane Shimizu, serialized in Kodansha's Morning). Set inside the human body, the film presents the “smallest story in the world” with the largest scale in Japanese cinema, blending the "human world" with the world inside the body.

The setting for this hard-working cell world is inside the body of a high school girl named Hina Urushizaki (played by Mana Ashida), who leads a healthy lifestyle and has a serious personality. Hina, who lost her mother at a young age, lives with her father Shigeru (Sadao Abe). While attending high school, she dedicates herself to household chores and is concerned about her father’s unhealthy habits. Inside Hina’s body, the environment reflects her lively nature, with a vibrant, European-like atmosphere where red blood cells, white blood cells (neutrophils), and many other cells work tirelessly to keep her healthy.

In the newly released stills, we see scenes from inside Hina’s body, featuring a red blood cell (played by Mei Nagano) dreaming of becoming fully-fledged, a white blood cell (played by Takeru Satoh) using the "receptor" on his hat to detect invading bacteria, and a red blood cell and neutrophil trapped in a colorful sphere.

Additionally, the stills show a killer T cell (Koji Yamamoto) battling bacteria, an NK cell (Riisa Naka) on guard with a weapon in hand ready to fight, a macrophage (Wakana Matsumoto) instructing red blood cells, a commander Helper T cell (Shota Sometani) oddly grinning and getting overly enthusiastic, and platelets (Maika Pugh) working diligently despite their tiny size. All of the cells are vividly portrayed as they carry out their vital tasks.
However, bacteria plotting to invade Hina's body begin to move... How will the cells rise to the challenge and protect Hina's health?
